This 10,500-square-foot, community-based Diagnostic & Treatment Center focuses on family medical and behavioral health services. The program consists of 25 exam rooms. Twelve will be for therapy/consult rooms for behavioral health, seven for pediatrics, and six for adult medicine. Support spaces include a waiting/reception area, M.A./nurse station, patient & MD consult rooms, medical and administrative staff offices, med distribution room, blood /lab, clean and soiled utility, storage, toilets, and a staff lounge.

The health center is located at ground level, with convenient front door drop-off and easy sidewalk accessibility. An art wall in the facility captures the historic evolution of Ezra Medical in a chronological photographic display. The lobby features a coffered ceiling and custom photo-printed glass.
